Hired Truck Requirements
Copies of the following documents/policies must be provided to Bomhak Trucking, LLC:
- W-9 Form
- Motor Carriers License (OCC AUTHORITY)
- Registration documents for ALL Trucks and Trailers
- Certificates of AUTO and GENERAL LIABILITY Insurance; a minimum of $1,000,000.00 Auto and General Liability Insurance coverage is required
- Certificate of WORKERS COMPENSATION Insurance; all hired Trucks ARE REQUIRED to carry Workers Compensation Insurance for ALL employees,
Bomhak Trucking, LLC MUST be the listed certificate holder AND additional insured on all insurance and workers compensation certificates. Must be on the certificate as follows:

- Owner-operator statement indicating that there are no other employees and that he/she is the only driver
- Affidavit of Exempt Status Under The Workers’ Compensation Act
